Release and Waiver of Liability, Assumption of Risk, and Indemnity Agreement
READ CAREFULLY BEFORE SIGNING:
By signing this agreement, you acknowledge and agree to the following terms:
1. Acknowledgment of Risks: I, the undersigned, understand and acknowledge that roller skating involves inherent risks, including but not limited to falls, collisions, equipment failure, and other dangers that could result in serious injury, permanent disability, or even death. I voluntarily assume all risks associated with participation in activities at Little Gliders Skate Zone, whether as a skater, spectator, or participant in other activities.
2. Release of Liability: I hereby release, waive, and discharge Little Gliders Skate Zone, its owners, employees, agents, and affiliates (collectively, “Released Parties”) from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my minor child(ren) as a result of participation in activities at Little Gliders Skate Zone.
3. Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, lawsuits, damages, costs, and expenses (including attorneys’ fees) arising out of or related to my or my minor child(ren)’s participation in activities at Little Gliders Skate Zone.
4. Medical Authorization: In the event of an emergency, I authorize Little Gliders Skate Zone to secure medical treatment for myself or my minor child(ren) and agree to assume all financial responsibility for such treatment.
5. Rules and Regulations: I agree to comply with all posted and communicated rules, warnings, and instructions from staff. Failure to follow these rules may result in immediate removal from the premises without refund.
6. Binding Agreement: This waiver is binding upon me, my heirs, executors, and assigns. I understand that by signing this document, I am waiving important legal rights.
Acknowledgment and Signature: I certify that I am at least 18 years old or the legal guardian of the minor(s) listed above. I have read this waiver, fully understand its terms, and sign it voluntarily.
